J Exchange Student Application for Form DS-2019

Important: for the J Exchange Student application, please attach the following documents:

• Copy of Passport.
• Financial Support document the following documents: personal or family funds (bank statements), letters of support from sponsoring employer, letters of scholarship, letters from government agencies, letters from sponsors, teaching or research assistantship from UTPA. These documents are needed to provide support for the amounts listed under Sources of financial support from the International Student Application and the amounts listed under Sources and proof of funding from the J Exchange Student Application.
• Medical Insurance should include the following information: dates of coverage, policy number and coverage information.

The following Complete Applications must be mailed, faxed or as scanned documents* to the Office of International Programs:

1. Student Exchange/Visiting Program Application (along with copy of current transcripts, letter of English Proficiency or TOEFL scores (minimum 500)
2. Student Information Form
3. Enrollment Form
4. International Student Application (For Undergraduate students) or International Student Application (For Graduate Students)
5. J Exchange Student Application for Form DS-2019* (along with copies of passport, financial documents and proof of medical insurance)
