IMPORTANT NOTICE REGARDING DEPENDENT INSURANCE COVERAGE ENROLLMENT PROCEDURES
Effective immediately, employees who will be making changes to dependent insurance coverage must submit required documents within 31 days of a life event (e.g., annual enrollment, birth of child, marriage, divorce, death, etc.). The documents will be required regardless of whether the dependent has previously been on any UT System insurance. Employees contemplating making changes should make sure that the necessary documents are available when making a change to dependent insurance coverage.

IMPORTANT: A Power of Attorney is not adequate legal documentation for establishing a Dependent relationship.
These requirements were established for the entire UT System to assure that uniform approved proof of eligibility documents are on file for all dependents on our insurance coverage.
